For all our working lives we have been constantly reminded how healthcare in developing countries is both challenging and substandard. The usual arguments are the lack of resources, the overwhelming numbers of poor patients who arrive at our hospitals with advanced disease, physicians who have been inadequately trained in models of care unsuited and irrelevant to our settings. Of late this list has been augmented by consideration of proft-driven private medical colleges where there is very little teaching, there is also competition from droves of unqualifed informal healthcare providers, little monitoring and accountability of the quality of services provided and, of course, ubiquitous corruption.
